Answer a few short questions and we will get back to you within 2 working days. Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 15.30 - 08.00. Please contact 111 during out of hours or 999 if this is an emergency.
Covid Vaccinations - The practice is vaccinating patients as part of the local group of practices and the majority of our appointments are at Church Farm in Ripley We do have occasional clinics at Parkside Surgery but please book into Church Farm if there are no appointments available at Parkside. We are following the government guidelines around vaccination cohorts and you will either be contacted by NHS England or by text from the practice when we are able to offer you the vaccination.
SECOND C0VID 19 VACCINATIONS
Please note we are unable to offer a 2nd vaccination appointment until 10 weeks following your first. You will automatically receive an invitation to book – there is no need to ring the practice unless it is over 11 weeks since your vaccination and you have not been contacted.

NHS App – Vaccine PassportEngland’s NHS app will be available to use as a vaccine passport from Monday 17/5/21, the government has said - but only for those who have had both doses of the jab. There is no requirement to contact the practice for enhanced access to view Covid Passport -please see more information by clicking HERE. A paper version will also be available - by calling 119 but not through a GP. This can be requested 5 days after your second vaccination.

Please note the surgery will be unable to take your requests after this date. To save you from waiting a call back system has been introduced which you can request by sending an email to our Medicines Order Line inbox.
